Nurse navigators and community navigators support income-eligible pregnant women and their babies. They assess family needs, plan services, make referrals, provide health education, conduct home visits, and advocate for clients. Nurse navigators may also conduct developmental screenings and perinatal depression screenings.
HRIF nurses work with doctors and intervention services to support families who have high-risk infants and have been referred to by local hospitals.
