Family Case Management / HRIF

Family case managers provide help and support to income-eligible pregnant women and babies. They assess each family’s needs, plan services, make referrals, provide health education, make home visits, and advocate for clients. Family case managers can also conduct developmental screenings, and perinatal depression screenings.

HRIF nurses can help coordinate care with doctors and other intervention services being used to assist families with High-Risk Infants that have been referred by local hospitals.
