Family Case Management (FCM) provides preventative care, developmental testing, immunizations, referrals, screenings, home visits, and follow-up for at-risk pregnant women, infants, and special needs children who meet income guidelines. The goal of the family case management program is to decrease infant mortality and morbidity (disease), improve pregnancy outcomes, and reduce the incidence of prematurity and low birth weight.
Case management coordinates and assures continuity of services (health, social, educational) for clients. It involves individualized assessment of needs, planning of services, referral, monitoring, and advocacy to assist clients in gaining access to appropriate services.
Additionally, FCM provides medical case management of eligible DCFS clients.
