Whenever a baby is born in a hospital, the hospital creates a certificate of live birth for that child and sends it to us. Parents of babies born at home must come to us (or go to a hospital) to register the birth and obtain the Birth Certificate. A notarized third-party statement of verification of the occurrence of a live birth is required. A witness fills out this form, verifying that the child was born to the woman who claims to be the child's mother.
How to obtain a copy of a Birth Certificate
Parents who are named on the Birth Certificate or legal guardians may call or write us for a copy of the child's Birth Certificate. Come to the Health District Main Building at 201 W. Kenyon Rd., Champaign, and visit the Registrar at the front office. You must bring a photo ID (preferably a driver's license) or court papers verifying that you are a legal guardian of the child. You will be asked to complete an application for birth Record. Birth Certificates cost $10 for the first copy, $2 for each additional copy per child. You may also write to request a Birth Certificate. You must provide a photocopy of a photo ID of a parent named on the Birth Certificate, or proof of legal guardianship. Fill out the application for birth Record and mail it in with your request.
