Acknowledgements

Health Map Online development was funded through a grant to the Champaign-Urbana Public Health District from the Centers for Disease Control and Prevention in 2009. The purpose of the grant was to improve public health response to disease outbreaks and to be more prepared for emergencies. The Health Map Online was only one part of that grant, with an emphasis on data processing and mapping. This was for the initial site, which is Phase 1, which was developed through April 2010.

Phase 2 was developed during the period from May 2010 – July 2011, with funding from the University of Illinois Center for One Health Illinois and a grant from the Champaign-Urbana Public Health District. During this time we have added a new region consisting of four counties in North-Central Illinois to Health Map Online: Winnebago, Boone, Ogle and Stephenson counties. In addition we have expanded the map information and have provided data download functions.

We will continue to add to and improve this site as funding allows.
