Well Child Services

Our Nurse Practitioner sees children for well child checks and school physicals. Now also offering sports physicals.

Well child visits allow medical staff to assess a child's development, administer any needed vaccinations, and discuss parents' questions or concerns about a child's health. The American Academy of Pediatrics recommends wellness exams at or around the following ages: 2 months, 4 months, 6 months, 9 months, 12 months, 15 months, 18 months, 24 months, 30 months, and then annually after age three.

How to obtain services

Locations:

201 W. Kenyon Rd., Champaign

Eligibility:

Any child through age 18.
Well child visits are available to WIC infants and children only.

Cost:

School and Sports Physicals are $30.
We bill Medicaid or insurance for well child visits, or see our Public Health Fees page for costs.

How do I get services?:

Call for an appointment.
Although appointments are recommended, we can often fit in wellness exams during WIC visits. Parents are encouraged to schedule school or sports physicals in advance, so that children will not risk being withdrawn from school.
