Illinois WISEWOMAN program

The Illinois WISEWOMAN Program's mission is to provide Illinois Breast and Cervical Cancer eligible participants, who are low-income, under- or uninsured 40-64 year old, with the knowledge, skills, and opportunities to improve diet, physical activity, and other lifestyle behaviors to prevent, delay, and control cardiovascular and other chronic diseases. Following lipid, glucose, and blood pressure measurements, IWP participants can choose one of four lifestyle interventions that best suits their risks and motivations. Unique to the IWP is Level 3 Lifestyle Intervention, which consists of four fun dietary and physical activity sessions designed to empower Illinois WISEWOMAN Program participants to prevent, delay, and control cardiovascular and other chronic diseases. How to obtain services

Eligibility:
You may be able to receive free services if you are a woman:
  • Enrolled in the Illinois Breast and Cervical Cancer Program through CUPHD
  • Living in Illinois
  • Without insurance
  • 35 to 64 years old
  • Household income at or below 250% of the federal poverty level, read the guidelines.

This program is FREE.
